Altitude Camp Consent and Waiver

This electronic form gives permission for the athlete named below to participate in the 2026 Bears Track Club Altitude Camp.

Camp is planned for Saturday, August 1 through Tuesday, August 4 in the Crested Butte / Mt. Crested Butte area. Vans leave from Palmer Ridge by the track at 11:00 AM Saturday. Lodging is split among several houses on Emmons Road, and all meals at camp are provided.

Camp Consent

Camp is currently planned for Saturday, August 1 through Tuesday, August 4 in the Crested Butte / Mt. Crested Butte area, Colorado. Camp may include team transportation, overnight lodging, meals, supervised free time, trail running, road running, hiking, strength and mobility work, and other team activities.

Activities may take place at altitude and on uneven outdoor surfaces. Coaches may adjust or stop activities based on safety, weather, health, or team needs.

I consent to the athlete participating in the 2026 Bears Track Club Altitude Camp.I authorize Bears Track Club coaches, staff, and approved adult designees to transport and supervise the athlete during camp.I have disclosed relevant health, medication, allergy, dietary, injury, or supervision concerns.I authorize Bears Track Club staff to seek emergency medical care if they believe it is necessary and a parent/guardian cannot be reached immediately.
